Please call CME at 989-831-2500 to schedule a 1 hour time slot between the hours of 4:30-7:00 p.m.
Please plan on being at CME at least 1 hour for your child to be screened and have their play group time.
Please park in the parent parking lot at CME on Pine St. in Sheridan.
Please enter through door #13 at CME in Sheridan.
Your child will be involved in a play group in a classroom while you are in the cafeteria going to stations and for the orientation.
Please prepare them for this and that you will pick them up after you're finished in the cafeteria.
You can start to enroll your Kindergartener (or new student) by doing the following:

1.) Click on the black box, "Final Forms".
2.) Fill in all of the information on every tab.
You are not finished until all of the tabs turn green on the left hand side.
3.) Make sure you click on "Submit Form" at the bottom of every form or it will not save.
4.) Every year after this, you will only need to update what has changed.
Please bring the following in to CME anytime from 8:00-3:00 (Monday-Friday):
-proof of residency (i.e. driver's license, utility bill, mail)
-original birth certificate
-current immunization form
If your child is not up-to-date on their immunizations, please schedule their appointment as soon as possible as the doctor's offices are full during the summer.
The green health form can only be filled out only by a doctor at your child's appt. I will post a white one so you can print it.
The oral health screening needs to be filled out by a dentist.
The following can be completed here at CME during Kindergarten Round Up:
-vision screening

-oral health screening

Make sure you bring all the required documents.
Certified birth certificate
Proof of residency (utility bill, voter registration, lease agreement, driver's license)
Immunization records
Custody paperwork (if applicable)
Copy of student's unofficial transcript, copy of last report card, and last IEP (if applicable) from previous school (recommended for placement)
